Data collected

The demo contact form only displays an in-browser success state. It does not send or store names, phone numbers, health information or other form data.

Traffic measurement

Depending on the project, privacy-conscious hosting statistics, Google Analytics or similar tools may be used to understand performance and broad visitor trends. Active tools and any legally required consent are disclosed for that project; this demo creates no health data, appointment records or patient profiles.
